Nomination forms are now available for the eighth annual civic awards ceremony sponsored by the Conejo/Las Virgenes Future Foundation.
The CLVFFie Awards, pronounced "Cliffy," recognize individuals and groups in the categories of the arts, business/professional, community service, education, public safety and youth leadership.
"This event is designed to give those nominated a special evening to know how much the community appreciates them," said Carl Herman, chairman of the foundation.
Applications are due June 30. The awards will be presented Nov. 22 at Los Robles Greens in Thousand Oaks, according to Fran Brough, the executive director.
One recipient in each category will be selected by secret ballot from among the finalists, she said. The winners will be revealed at the dinner celebration.
Applications can be obtained by calling the foundation's office in Calabasas at 818-880-1054 or from the foundation's Web site at http://www.clvff.org.
Although space at the awards dinner is limited, community members can call the foundation to receive an invitation.
